Phrenic nerve reconstruction by intercostal nerve neurotization on pigs

چکیده
Background: Our aim was to investigate the motor outcome, effects on the diaphragm muscle, and delimit the regained nerve conduction capacity of the transected phrenic nerve, following its reconstruction to prepare the subjects to electrophrenic respiration. Methods: All one-year-old 12 healthy pigs (German Landrace) weighing 45 to 50 kg were used in the study. Three groups, each of which consisted of four subjects were composed; control group, anastomosis group, and anastomosis plus pericardial flap group. Initial nerve conduction studies were applied and data were recorded. All the subjects encountered left phrenic nerve transection and except the control group, distal stumps were immediately repaired with intercostal nerve neurotization and coaptation. One subject from each group was sacrificed at 1 h, 2 h, 6 h and 4 weeks after surgery, following final nerve conduction assessments for comparison. Results and conclusions: Harvested nerve distal stumps and diaphragm muscles were examined for histopathological changes. The results yielded that reconstructed nerves responded to stimulation in the end of four weeks and diaphragm was excitable thereafter. Wrapping the repair site with pericardial fat tissue did not prove to be superior to nerve repair alone. More detailed experimental studies are needed to find out the best estimates of timing for repair and neurostimulation.

Breast Reinnervation: DIEP Neurotization Using the Third Anterior Intercostal Nerve
BACKGROUND The purpose of this article is to evaluate a new method of DIEP flap neurotization using a reliably located recipient nerve. We hypothesize that neurotization by this method (with either nerve conduit or direct nerve coaptation) will have a positive effect on sensory recovery. METHODS Fifty-seven deep inferior epigastric perforator (DIEP) flaps were performed on 35 patients. Neurot...

PURPOSE We conducted a clinical study to evaluate the effects of neurotization, especially comparing the total contralateral C7 (CC7) root transfer to hemi-CC7 transfer, on total root avulsion brachial plexus injuries (BPI). METHODS Forty patients who received neurotization for BPI were enrolled in this prospective study. Group 1 (n = 20) received hemi-CC7 transfer for hand function, while gr...
متن کاملRehabilitation, Using Guided Cerebral Plasticity, of a Brachial Plexus Injury Treated with Intercostal and Phrenic Nerve Transfers
Recovery after surgical reconstruction of a brachial plexus injury using nerve grafting and nerve transfer procedures is a function of peripheral nerve regeneration and cerebral reorganization. A 15-year-old boy, with traumatic avulsion of nerve roots C5-C7 and a non-rupture of C8-T1, was operated 3 weeks after the injury with nerve transfers: (a) terminal part of the accessory nerve to the sup...
متن کاملReinnervation of the paralyzed diaphragm: application of nerve surgery techniques following unilateral phrenic nerve injury.
BACKGROUND Unilateral phrenic nerve injury often results in symptomatic hemidiaphragm paralysis, and currently few treatment options exist. Reported etiologies include cardiac surgery, neck surgery, chiropractic manipulation, and interscalene nerve blocks. Although diaphragmatic plication has been an option for treatment, the ideal treatment would be restoration of function to the paralyzed hem...
متن کاملNeurotization of the phrenic nerve with accessory nerve for high cervical spinal cord injury with respiratory distress: an anatomic study.
AIM High cervical spinal cord injury is associated with high morbidity and mortality. Traditional treatments carry various complications such as infection, pacemaker failure and undesirable movement. Thus, a secure surgical strategy with fewer complications analogous to physiological ventilation is still required. We hope to offer one potential method to decrease the complications and improve s...
